EM Academy 2010
In partnership with the Florida Division of Emergency Management (FDEM) and the Florida Emergency Management Association (FEPA), CDRP was pleased to be a part of the Basic EM Academy for 2010.
As part of the ten day Academy, CDRP taught two days on the Foundations of Emergency Management. This course was a highly condensed version of the 3 hour PAD 4391 "Foundations of Emergency Management" taught by Center staff at FSU. As a shortened version of this academic course,
Academy students were given the opportunity to complete these 16 hours of instruction for undergraduate credit. To take advantage of this opportunity, Academy students must complete three forms and fax them to the Registrar at FSU.

Instructions
Special Student Application and Residency
Complete and sign the Special (Non-Degree Seeking) Student application form
including the residency affidavit. To claim Florida residency, you should provide two proofs, such
as your Florida driver's license number and original date of issue, your
voter registration number and issue date, and/or your vehicle
registration number. The documents must have an issue date at least one
year prior to the request for FSU admission to meet the one-year
residency requirement.
Course Registration Form
Complete the Continuing Studies Credit Registration form
with your personal information on the top half of the form. Then,
add Course Reference Number, Course Prefix, Course Number, Course
Section and Course Hours, leaving the rest blank. This information for
the Spring 2010 semester is as follows:

Course Section: 2
Course Hours: 1
Remember to sign and date the form at the bottom.

Eligibility and Deadline
Only students from the FDEM/FEPA EM Academy 2010 (Basic Course) are eligible for this credit.
The deadline to have all paperwork received at FSU is Friday, 16 APRIL 2010 at 1600. Sorry, no exceptions.
